In this article, we explain how rheumatoid arthritis develops, what may cause it, how it is diagnosed and treated, and what role rheumatoid factor plays in the diagnostic process.

What Is Rheumatoid Arthritis?

The Nature of the Disease and How It Develops

Rheumatoid arthritis is a chronic disease caused by an abnormal immune response. The immune system mistakenly attacks the body’s own tissues, particularly the synovial membrane, which lines the inside of the joint and helps produce synovial fluid.

Normally, the immune system protects the body against infections. In rheumatoid arthritis, however, it begins to recognize the body’s own tissues as a target. This triggers inflammation that may persist for months or years.

Inflammatory cells accumulate in the affected joints. Swelling, pain, and a burning sensation may develop. Movements become more limited and cautious, and patients begin adapting their everyday activities, work, and physical exercise to the condition of their joints.

Persistent inflammation causes the synovial membrane to thicken. Over time, the cartilage, bone, ligaments, and tendons may also become damaged. Without proper disease control, rheumatoid arthritis can lead to joint deformity, reduced hand strength, and permanent limitation of movement.

How Rheumatoid Arthritis Differs from Osteoarthritis

Osteoarthritis is primarily associated with cartilage degeneration, mechanical overload, and age-related changes. Pain caused by osteoarthritis usually becomes worse after activity, while stiffness after rest tends to be brief.

Rheumatoid arthritis develops differently. It is caused by an autoimmune attack on the joints, so pain is often more pronounced in the morning and joint stiffness lasts longer. A typical feature is that the hands and feet may feel more mobile after movement, while prolonged stiffness returns after rest.

Causes of Rheumatoid Arthritis

Autoimmune Processes in the Body

The main mechanism of rheumatoid arthritis is immune system dysfunction, in which the body attacks its own tissues. T lymphocytes, B lymphocytes, cytokines, autoantibodies, and other components of the inflammatory response are involved in this process.

The synovial membrane becomes an active source of inflammation. It thickens, produces inflammatory mediators, and maintains a reaction that gradually damages the joint from within.

The autoimmune process is not limited to the joints. In some patients, rheumatoid arthritis may also affect the eyes, skin, lungs, cardiovascular system, blood vessels, and nerves.

Genetic Predisposition

Genetic predisposition increases the risk of developing rheumatoid arthritis, but it does not mean that the disease will necessarily be inherited.

Smoking is an important modifiable risk factor. It changes proteins in the tissues of the respiratory tract and is associated with the development of antibodies to cyclic citrullinated peptides.

These antibodies are often detected in people with rheumatoid arthritis. If the disease has already been diagnosed, smoking may worsen its course and make inflammation more difficult to control.

Hormonal changes can also influence the immune system. Rheumatoid arthritis is diagnosed more often in women, and the first symptoms may appear after childbirth, during perimenopause, or alongside other endocrine changes.

During these periods, immune regulation may become less stable. A predisposition to autoimmune inflammation may become more apparent, and a previously silent process may progress to noticeable symptoms.

Symptoms of Rheumatoid Arthritis

Early rheumatoid arthritis may present as stiffness in the hands, finger pain, swelling of the small joints, and unexplained fatigue. A typical warning sign is morning stiffness that lasts longer than 30 to 60 minutes and improves with movement.

The main symptoms include pain, swelling, local warmth, reduced grip strength, and difficulty performing fine hand movements. Everyday activities such as fastening buttons, opening a bottle, holding a pen, using a phone, or typing for long periods may become more difficult.

Without treatment, rheumatoid inflammation gradually damages the structures of the joint. The cartilage becomes thinner, bone erosions develop, ligaments weaken, and tendons may shift as the joint becomes deformed.

What Is Rheumatoid Factor?

Rheumatoid factor is an autoantibody that reacts with the body’s own proteins.

In rheumatoid arthritis, this reaction is directed against part of the immunoglobulins, which are protective proteins normally involved in fighting infections. Most often, doctors assess IgM antibodies directed against the Fc portion of IgG.

An elevated result may indicate abnormal immune system activity. Instead of responding only to external threats, the immune system involves the body’s own structures in the inflammatory process. This can contribute to persistent inflammation and increase the risk of joint damage.

Rheumatoid factor alone does not confirm the diagnosis. Its result must be interpreted together with other information, including:

When prolonged morning stiffness and symmetrical swelling of the small joints are also present, an elevated rheumatoid factor significantly increases suspicion of rheumatoid arthritis.

A normal rheumatoid factor result does not rule out the disease. The main treatment strategy is known as treat-to-target. This means that the doctor regularly assesses disease activity and establishes specific treatment goals. If the target is not achieved, the treatment plan is adjusted.

Methotrexate is often used as an initial treatment for active rheumatoid arthritis. Before it is prescribed, the doctor assesses liver and kidney function, complete blood count results, associated infections, pregnancy, or plans for pregnancy.

Anti-Inflammatory Medications and Disease-Modifying Therapy

Nonsteroidal anti-inflammatory medications can reduce pain and stiffness, but they do not stop joint destruction or change the course of the autoimmune disease.

Disease-modifying therapy targets the underlying mechanism of rheumatoid arthritis. It suppresses abnormal immune system activity, reduces inflammation, and helps protect joint structures from further damage.

Glucocorticoids can rapidly reduce active inflammation. However, prolonged use without strict indications can cause significant adverse effects. These medications are generally used as temporary treatment while disease-modifying therapy takes effect or to control a flare-up.

If conventional disease-modifying medications do not provide adequate control, the doctor may prescribe biological or targeted synthetic medications. This treatment requires regular laboratory monitoring, assessment of infection risks, and observation for possible side effects.

Rheumatoid arthritis cannot be prevented completely. Some risk factors cannot be changed, including genetics, sex, age, and individual characteristics of the immune system. Therefore, preventive measures cannot guarantee that the disease will not develop.

One of the most important steps is to stop smoking. Tobacco smoke increases the risk of autoimmune inflammation and can worsen the course of existing rheumatoid arthritis. People with a family history of autoimmune disease or previously detected autoantibodies should be particularly attentive to possible symptoms.

It is also important not to ignore early warning signs. Prolonged morning stiffness, swelling of several joints, and symmetrical pain in the hands or feet are reasons to consult a doctor rather than wait for several months. Starting treatment early reduces the risk of bone erosions, joint deformities, and permanent loss of function.

In this sense, prevention begins with timely diagnosis. The earlier the rheumatoid process is identified and appropriate treatment is started, the greater the chance of preserving mobility, hand strength, and quality of life.

The most typical signs include symmetrical pain and swelling of the small joints, prolonged morning stiffness, weakness, fatigue, and gradually reduced mobility. If these symptoms continue for more than a few weeks, delaying medical evaluation is not recommended.

Rheumatoid factor is useful in diagnosis, but it cannot confirm the disease on its own. The doctor evaluates the entire clinical picture, including anti-CCP or ACPA, C-reactive protein, erythrocyte sedimentation rate, X-ray or ultrasound findings, and the functional condition of the joints.

Modern treatment methods can reduce inflammation, protect the joints from further damage, and help patients return to their usual activities. The best outcomes are associated with early treatment, regular monitoring, and consistent adherence to medical recommendations.
