Osteosynthesis (treatment of bone fractures)

#method

Osteosynthesis is an effective method for treating complex fractures. It involves fixing bone fragments in an anatomically correct position. For this purpose, special metal devices are used — either external or internal. Plates, rods, pins, screws, and other implants stabilize the fragments and ensure proper bone healing.

What is osteosynthesis and when is it needed?

Osteosynthesis is a surgical procedure in which an orthopedic surgeon repositions fractured bone fragments and stabilizes them in the correct alignment. Further recovery depends on the body’s natural healing processes until complete bone union is achieved.

This technique is used in complex cases when cast immobilization is not sufficient. It is indicated for the following types of fractures:

  1. Displaced fractures. Significant displacement may impair proper healing or lead to malalignment. Such injuries require stable internal fixation.
  2. Comminuted fractures . Multiple bone fragments complicate healing. Implants help maintain alignment and reduce the risk of complications such as non-union.
  3. Intra-articular fractures (restoration of the articular surface). When a fracture involves a joint, surgical precision is critical. Improper healing may lead to osteoarthritis and reduced mobility. Accurate restoration of the joint surface and stable fixation help prevent these outcomes.
  4. Open fractures. Associated with soft tissue injury and exposure of bone, that increases the risk of complications. Surgical stabilization is often required after proper wound management to reduce infection risk.

Contraindications to this procedure

Surgery is not performed in certain cases:

It should be noted that osteosynthesis is an invasive procedure and may involve postoperative discomfort and a longer recovery period in some cases.

How is osteosynthesis performed?

Osteosynthesis is performed under general or regional anesthesia. The surgeon makes an incision to access the fracture site. Preoperative imaging (X-ray, CT, or MRI) is used to plan the procedure.

Bone fragments are carefully repositioned into their correct anatomical alignment. After open reduction, they are fixed using appropriate implants:

  1. Plates and screws are used for fractures of long bones such as the femur, tibia, and forearm;
  2. Intramedullary nails (rods) are inserted into the bone canal and secured with locking screws;
  3. Pins, wires, screws, or cerclage fixation are used for smaller bones, such as those of the hand or foot, or in complex fracture patterns.

Intraoperative imaging (fluoroscopy) is used to confirm correct positioning. After fixation, the wound is closed and a sterile dressing is applied. In some cases, additional immobilization with a cast or orthosis may be used, although it is often not required.

Recovery after osteosynthesis

After surgery, the patient remains under medical supervision for several hours to several days, depending on the condition. Additional treatment, such as antibiotics, may be prescribed, especially in cases of open fractures.

  1. Early mobilization. If there are no contraindications, gentle movement of the affected limb may begin as early as the day after surgery. This promotes recovery and helps prevent stiffness.
  2. Physical therapy. Individually selected exercises strengthen muscles, improve ligament elasticity, and restore joint function.
  3. Physical therapy modalities. The clinic offers methods such as high-intensity laser therapy and focused shockwave therapy, which may support tissue healing and help prevent complications.

Bone healing typically occurs within 6–12 weeks. However, full rehabilitation may take 6–12 months. Metal implants can remain in the body long-term; if necessary, they may be removed after 1–2 years.
