Treatment of calcific tendinitis

#method

Calcific tendinitis is a condition in which calcium deposits form within the tendons of the shoulder. It is commonly associated with shoulder pain, reduced range of motion, discomfort, and muscle weakness. Symptoms may worsen over time but can be effectively managed with appropriate treatment.

What is calcific shoulder tendinitis?

Calcific tendinitis is characterized by the deposition of calcium hydroxyapatite crystals within the tendons of the rotator cuff. The supraspinatus tendon is most commonly affected. Inflammation and mechanical irritation lead to pain and impaired shoulder function.

The condition typically progresses through several stages:

  1. Pre-calcific phase: structural changes occur in the tendon without significant symptoms;
  2. Calcific phase: calcium deposits form and symptoms develop;
    • Formative phase: active deposition of calcium;
    • Resting phase: deposits become stable;
  3. Post-calcific phase: tissue remodeling and gradual restoration of function.
    • The resorptive phase. It is characterized by gradual resorption of deposits and increased pain.
    • Resorptive phase: breakdown of deposits, often accompanied by increased pain;

Symptoms of calcific tendinitis

  1. shoulder pain that worsens with movement, especially when lifting the arm;
  2. reduced range of motion;
  3. night pain that may interfere with sleep;
  4. weakness in the affected arm;
  5. occasional swelling or redness (rare).

  1. nonsteroidal anti-inflammatory drugs (NSAIDs) to relieve pain and inflammation;
  2. physiotherapy, including shock wave therapy or Super Inductive System (SIS) therapy;
  3. ultrasound-guided needling and lavage (barbotage) to remove calcium deposits;
  4. corticosteroid injections for inflammation control.

Surgical treatment

 Surgery is considered when:

Arthroscopic removal of calcium deposits is performed using minimally invasive techniques. Small instruments are inserted through tiny incisions, allowing precise removal with minimal soft tissue damage. If necessary, tendon repair is performed.

Effectiveness of treatment

 Appropriate treatment is effective in the majority of cases. A combination of medication, physiotherapy, and barbotage often provides rapid symptom relief.

If conservative therapy is unsuccessful, arthroscopic treatment offers reliable results. Proper rehabilitation reduces the risk of recurrence and restores shoulder function.
