BMAC regenerative therapy

How does regenerative therapy (IUD) work?

Injections of red bone marrow concentrate activate natural tissue regeneration mechanisms. The concentrate contains:

• Other cell fractions.

The main effect in the joint is immunomodulation, reduction of inflammation, improvement of microenvironment for regeneration. This reduces pain and improves mobility. In what cases does the IUD procedure help?

BMAC is indicated for injuries, degenerative changes and the consequences of excessive stress, in particular:

• osteoarthritis of the knee, hip, shoulder joint (II—III degree);
• chondropathy and cartilage damage;
• partial ruptures of ligaments and tendons;
• “tennis player's elbow”, “runner's knee”, achillobursitis;
• slow fusion of fractures and microcracks;

Contraindications to IUD injections

IUAS therapy is not used in the following cases:

• active infectious processes in the body;
• oncological diseases;
• blood clotting disorders or the need for constant intake of anticoagulants;
• severe chronic conditions (decompensated cardiac, renal, pulmonary disorders);
• pregnancy and lactation.

Mechanics of the procedure

Initially, under local anesthesia, aspiration of the bone marrow from the wing of the ilium is performed. The collection lasts 10-15 minutes. The resulting material is then centrifuged, releasing a concentrate of red bone marrow aspirate — a fraction enriched with active cells and factors.

The resulting suspension is injected into the affected area — under ultrasound control. This can be a joint, a place of attachment of tendons or an area of bone damage. The duration of the entire procedure is up to 1 hour. Hospitalization is not required.

After the procedure, the patient receives instructions on limiting the load, the allowed movements and the schedule of control intake.

What are the advantages and effect of this method?

Unlike symptomatic injections of hyaluronic acid or corticosteroids, BMAC (bone marrow aspirate concentrate) aims to trigger tissue repair. The main advantages are as follows:

• stimulates the restoration of cartilage, tendons and bones;
• reduces pain and inflammation without drugs;
• does not cause allergies (own material is used);
• compatible with physiotherapy and other methods;
• suitable for patients who are contraindicated in surgery.

In 2021-2024 studies, up to 80% of patients noted improvement in joint function and reduced pain 4—8 weeks after the procedure. Efficacy depends on the initial state of the tissue and the number of stem cells in the concentrate.
